1. Health Case Management & Nursing Delegation
Our Health Case Management and Delegation services ensure professional nursing oversight while supporting individuals to live safely and independently in their communities.
The purpose of this service is to:
Maintain and promote the health, safety, and well-being of individuals served by DDA
Ensure safe and accurate administration of medications and treatments
Provide professional nursing oversight for delegated health-related tasks
Prevent avoidable medical complications, hospitalizations, and medication errors
Support independence while ensuring appropriate healthcare support
Nurse Consultation Services
These services focus on health promotion, disease prevention, and care coordination and do not include hands-on nursing or task delegation unless authorized under a separate service.
The purpose of Nurse Consultation is to:
Promote optimal health outcomes for individuals served by DDA
Provide clinical education and guidance to individuals, families, and providers
Support informed decision-making related to health conditions and treatments
Reduce preventable medical complications and hospitalizations
Support person-centered planning and coordinated service delivery
